POMS is a survey that helps the research team gain more insights into understanding prescription opioid use.

There will be no cost to anyone for participating in this study.

Only a small group of researchers with appropriate training will be able to link your name and other personal information (e.g., birthday) to your responses on the survey or your health information. Your doctor will NOT be able to link the information you provide on the survey with your electronic health record information. All of your data will be protected and stored by supported firewalls to minimize the risk that your data would be stolen and made public.
